Hospice Physicians - Our team of physicians is experienced in managing terminally ill patients. They actively participate in admission and eligibility decisions. The hospice medical directors are responsible for treating physical symptoms and managing terminal illnesses and any conditions related to the disease. If your primary physician is not available, then the hospice physician is there to meet your medical needs.
Registered Nurse - A Registered Nurse is typically the first person you will meet on the hospice team. The RN will provide ongoing physical assessments and communicate with your loved one’s physician.
CNA - The hospice CNA assists patients with activities such as personal hygiene, dressing, and providing companionship. They assist with teaching families how to provide care and help patients maintain their dignity.
Bereavement and Spiritual Support Team- provideone-on-one or group therapy to assist our chaplains.
Chaplains- Our non-denominational Chaplains are available to assist you and your family with spiritual concerns during a terminal illness. They provide regular therapeutic visits, one-on-one counseling, guidance in arranging funeral services.
Social Workers- The Hospice Social Worker assists with end-of-life wishes and your family in dealing with the complex emotions related to you or your loved one’s illness. Social workers help you identify community resources that may be available. They assist with funeral planning and assist with placement in facilities if additional care is needed.
